Discounted Lifestyle Card Holders
HEALTH REWARDS
Health Rewards terms and conditions apply in full. If there
is a dispute regarding the implementation or interpretation
of these terms and conditions the decision of Health Rewards
will be final.
Membership of Health Rewards - Platinum
Status
A Health Rewards Platinum Status membership fee
(inclusive of VAT) will be payable by debit order.
On each enrolment anniversary date, Health Rewards will
automatically renew the membership and debit the nominated
account in the amount of the applicable membership fee.
Membership Fee in arrears
When a member's membership fee for Health Rewards
and/or contribution payment to the Medical Scheme falls
into arrears, the allocation of "rewards" will be suspended.
When the overdue amount is paid, all "rewards"
will be re-instated. If a member falls into arrears three
times - not necessarily on the same account, Health Rewards
reserves the right to cancel his or her membership.
Changes or
cancellation by Health Rewards
Health Rewards reserves the right to change or cancel Health
Rewards or any of its rules, clinical criteria, basis of
"reward" allocation, payout ratios, or parts thereof
without prior notice. However, Health Rewards will endeavour
to inform members of any changes.
Cancellation of Membership by Health
Rewards Member.
Membership of Health Rewards may be
cancelled only after 12 month period. Simply by notifying Rewards in
writing. Send your letter to Health Rewards: 53-55 Blaauwberg Road,
Vision House Unit 9, Bloubergstrand, 7441, Cape Town, South
Africa.
Terms and Conditions applicable
Members should always ensure that they refer to the
most current communication regarding Programme rules. Those are the
terms and conditions that will apply, not withstanding any earlier
correspondence, brochures, or terms and
conditions.
Rewards
All "rewards" are subject to availability.
In sourcing guaranteed free rewards The Rewards Company
endeavours to ensure that its suppliers are reputable and
that the guaranteed free rewards are of the highest quality.
However The Rewards Company give no warranties of any nature
whatsoever as to the condition, suitability, quality, availability,
fitness or safety of any guaranteed free rewards, and The
Rewards Company accept no liability whatsoever for any claims,
loss, damage of whatsoever nature arising or in connection
with the supply of any guaranteed free rewards.
General
If at any time a dispute
or claim arises out of or relates to these Terms and Conditions,
either the COMPANY or the MEMBER may give notice to this effect to
the other in writing and such dispute or claim, which cannot be
settled amicably by the parties shall be settled by arbitration in
accordance with South African Law and the award resulting therefrom
shall be final and binding upon the parties hereto.
No Agent or employee of the COMPANY other than a
director of the COMPANY may alter or vary these Terms and
Conditions. Any such alteration or variation must be reduced to
writing and shall only be effective when signed by a director of the
COMPANY.
No indulgence granted by the COMPANY shall
constitute a waiver of any of the COMPANY'S rights, accordingly the
COMPANY shall not be precluded as a consequence of having granted
such indulgence from exercising any rights against the MEMBER which
may have arisen in the past or which may arise in the
future.

COMPETITIONS & PROMOTIONS
Gym & Fitness
Club Activity Rewards Program
- Members will be required to sign a 2 year Health Rewards
Platinum Members agreement.
- Discount Value limited to a maximum of R135.00 per month per
member.
- Cash will only be paid out upon usage confirmation received.
Usage not claimed after 30 days will not be paid out.
- Organizers reserve the right to reject any application / entry
if the transaction falls outside the time period, is perceived to
be an invalid transaction and for any other reasonable reason.
- The decision of the Internal Auditor is final. Draw dates will
take place no later than the 7th of each month and each winner
will be notified.
Life CoverClaims must be submitted within 4
months of date of death.
Members over the age of 60 will pay the premium rate as indicated
on the rates table -

DISCOUNTED
LIFESTYLE CARD HOLDERS
Only the principal member can make use of the Discounted
Lifestyle services by quoting their Discounted Lifestyle
membership number.
In the event of a
member abusing any of
the Discounted Lifestyle benefits, Discounted Lifestyle
reserves the right to suspend or cancel the member's membership
with immediate effect.
Discounted lifestyle
cannot be held liable for any
damages arising from incorrect information supplied
to any members. There is no charge for information or quotations
when calling the Discounted Lifestyle services. Discounted
lifestyle will not be held liable for the payment of any
external or third party, that may arise as a result of calling
the Discounted Lifestyle call centre.
The use of
Discounted Lifestyle services
shall be at the member's own initiative and risk. Discounted
Lifestyle makes no representations, gives no warranty of
any kind expressed or implied, regarding the suitability
of any Discounted Lifestyle products or services. Discounted
Lifestyle shall not be liable for any direct, indirect,
incidental, special or consequential damages arising from
the use of Discounted Lifestyle. Discounted Lifestyle members
or prospective members agree also that neither Discounted
Lifestyle nor any of the listed service providers or organizations
can be held liable under any circumstances whatsoever whether
by willful act , negligence, default or otherwise.
